NTSB: Amazon Cargo Plane’s Speed Brakes and Thrust Reversers Were Not Deployed Before Miami Runway Crash

  • NTSB lead investigator Chihoon Shin said Tuesday that data from one of the Boeing 767’s flight data recorders shows no evidence that either the speed brakes (aerodynamic drag panels) or thrust reversers (systems that redirect engine airflow to decelerate) were deployed before the Amazon Air cargo plane overran the Miami International Airport runway on Sunday.
  • The non-deployment of both systems is a significant investigative finding: speed brakes and thrust reversers are the primary deceleration tools available to a flight crew after touchdown, and their combined absence means the aircraft was relying only on wheel brakes to slow down — dramatically reducing stopping capability on what investigators will now examine for evidence of excessive approach speed, late touchdown, or crew incapacitation.
  • The crash, which killed five people when the Boeing 767 careened off the runway and into nearby vehicles, is Amazon Air’s deadliest accident and comes as the company’s air cargo fleet has grown rapidly to support Prime delivery commitments — raising questions about fleet maintenance, crew training standards, and whether the rapid expansion of third-party air cargo operations has outpaced safety oversight.
  • The NTSB is in early investigative stages and has not assigned probable cause; investigators will analyze both flight data recorders (the data recorder and voice recorder), review air traffic control communications, examine the aircraft’s maintenance history, and assess crew qualifications and fatigue levels — a process typically taking 12-18 months for a full report.

What Happened?

An Amazon Air Boeing 767 cargo flight overran a runway at Miami International Airport on Sunday, careening off the end of the runway and crashing into nearby vehicles, killing five people. At a Tuesday briefing, NTSB lead investigator Chihoon Shin disclosed that data from one of the aircraft’s black boxes showed no evidence that either the speed brakes or thrust reversers were deployed before the plane left the runway. Speed brakes are wing panels that create aerodynamic drag to slow the aircraft; thrust reversers redirect engine exhaust forward to reduce speed after landing. Both are standard, expected elements of a normal landing rollout.

Why It Matters?

The non-deployment finding immediately focuses the investigation on what was happening in the cockpit during the final approach and landing rollout. There are three broad categories of explanation: crew failure to deploy the systems (intentional or through distraction), a technical malfunction preventing deployment, or an anomalous landing scenario (very high approach speed, late touchdown) that overwhelmed stopping capability even with full systems available. The distinction matters enormously for Amazon’s legal and regulatory exposure — a maintenance failure is different from a crew performance failure, which is different from a systemic training issue. For Amazon Air, which has rapidly expanded its fleet to over 100 aircraft, the accident will invite regulatory scrutiny of whether growth has been matched by commensurate investment in safety infrastructure.

What’s Next?

The NTSB will release a preliminary report within 30 days providing factual information without probable cause determination. The full investigation — covering both recorders, ATC communications, maintenance logs, crew records, and toxicology — typically takes 12-18 months. The FAA may conduct a separate review of Amazon Air’s certificate holder (Air Transport International) pending findings. Amazon faces civil litigation from the families of the five victims. The broader policy question is whether the FAA’s oversight framework for rapidly growing Part 121 cargo operations — which have different rest rules and training requirements than passenger carriers — is adequate given the pace of e-commerce-driven air cargo expansion.
